Patient Forms

Upon entering our office, we will have you complete some paperwork.  If you would like to save 20 minutes on your first appointment, we request that you fill out the patient forms ahead of time.  They are located here.  The paperwork provides us with general information about yourself and your condition.

Consultation

Next you will have a consultation with one of the doctors to discuss your health-related problems.  Your first visit is designed to learn more about you, your condition and your expectations to determine whether chiropractic care will you meet your goals.

Examination

After your consultation, one of the doctors will perform a complete chiropractic examination testing your reflexes, your ability to turn and bend as well as other standard neurologic, orthopedic, postural, and physical examinations.

X-Ray Studies

Your specific condition may or may not require us to take x-rays.  X-rays help us rule out more serious conditions or assist us with our treatment.  If you are pregnant, x-rays will not be necessary for chiropractic treatment.

Report of Findings

Your examination and x-rays will be reviewed by your chiropractor.  At the end of your Report of Findings, the following questions will be answered:   

Can chiropractic help me?

How long will it take to get better?

How much will it cost?

Treatment

Patients will be provided with treatment at the conclusion of the first or second appointment. This may include spinal adjustments, therapeutic exercise, and/or soft tissue therapies.  Our spinal adjustments can be customized to your liking;  Kaufman Chiropractic Clinic offers hands-on adjusting as well as safe, gentle instrument guided adjustments.
